Offline Registration for NCDD 2004
Below are three downloadable versions of the registration form: a Microsoft Word XP document (for those with recent versions of Word), a Microsoft Word 97 document (for those with older versions of Word), and a PDF file (for printing and manually writing on).
If at all possible, please use one of the Word versions so that you can type in your responses and email us the completed form.
The Registration Form
When downloading these documents, be sure to right-click and save them onto your computer before completing the forms.

* The form will not work properly if you open it in your browser instead of saving it onto your computer. Be sure to "right-click" or "option-click" on the link; then select "save target as", "save as," etc.; then save onto your computer; then open that saved document to complete the form.
** You can also use this version if you intend to open the file in a program that supports MSWord documents such as OpenOffice, WordPerfect, etc.
Submitting your Registration Form

Payment Options
After you complete the registration form and know the total amount that is due, you may either use your credit card at PayPal or mail us a check or money order. If you choose to mail us your payment, send it to the address above with a printed copy of your registration form if possible (but please still email a copy to ).
To make a credit card payment, go to www.paypal.com and either log in or click on "Sign Up" if you don't already have a PayPal account. After you are signed in, click on "Send Money." Type the following email address into the box that asks for the Recipient's Email: [email protected]. If you need a printed receipt, you will find a record of your transaction under "My Account" at PayPal.
Registrants will be responsible for arranging and paying for their lodging during the conference. Details about local hotel options (ranging from approximately $50 to $130 per night) can be found here. People who are registered will also receive hotel details via email.
Confirmation of Registration and Payment
We will confirm receipt of your registration form within one week of the date we receive it. The confirmation, which will be sent to you via email, will include information about whether or not we have received your payment yet. Please note that we will not be able to save a spot for you at the conference until payment has been confirmed.
