Registration & Scholarship Forms
Register now to ensure your place at the first National Conference on Dialogue & Deliberation! Submission instructions are included on the registration form and at the bottom of this page.
The registration fee is $200. Other costs (lodging, parking, travel, etc.) are addressed on the Registration Form and on the FAQ page of this website.
The registration deadline is September 1.
*It is recommended that you save this document onto your computer before completing it. If you prefer, you can also print out the document and fill it out manually or with a typewriter.
Now that the deadline for scholarships have passed, we are no longer accepting scholarship applications.
The National Conference on Dialogue & Deliberation has a limited amount of scholarship funds available. If you or your organization are unable to cover the entire registration fee, lodging costs and/or travel costs associated with attending the conference, please submit this application with your completed registration form.
Submitting the Forms
The completed forms can be submitted via email or mail to Sandy Heierbacher, conference director. Email the forms as attachments to (registration fees will still have to be mailed). Or you can mail the forms and check to:
