Offline Registration for NCDD 2006 in San Francisco
We have included a MSWord version to use for offline registration. Although we encourage you to use the online version, we understand that some computing environments may have trouble with online forms. If you cannot use the following format, please contact us for further options. Please Note: the online systen can be used to generate a printable invoice which you may include with a check.
Please also note that the Word version of the registration form states that the early registration rate of $300 ends on June 15, which is not accurate. We have extended that rate until June 26.
The Registration Form
When downloading the following documents, be sure to right-click* and save it onto your computer before completing the forms.
* The form will not work properly if you open it in your browser instead of saving it onto your computer. Be sure to "right-click" or "option-click" on the link; then select "save target as", "save as," etc.; then save onto your computer; then open that saved document to complete the form.
** You can also use this version if you intend to open the file in a program that supports MSWord documents such as OpenOffice, WordPerfect, etc.
Submitting your Registration Form

After you complete the registration form and know the total amount that is due, you may either use your credit card at PayPal or mail us a check or money order. If you choose to mail us your payment, send it to the address above with a printed copy of your registration form if possible (but please still email a copy to ).
To make a credit card payment, go to www.paypal.com and either log in or click on "Sign Up" if you don't already have a PayPal account. After you are signed in, click on "Send Money." Type the following email address into the box that asks for the Recipient's Email: [email protected]. If you need a printed receipt, you will find a record of your transaction under "My Account" at PayPal.
Conference participants are responsible for arranging and financing their own lodging and flights. We have negotiated a great rate of only $135 a night for you at the Renaissance Parc 55 where the conference will be held. When making lodging and flight reservations, consider that the conference will begin on the morning of Friday, August 4th and will end on Sunday, August 6th by 4:00pm. Also, most pre-conference activities begin first thing Thursday morning, August 3rd.
Confirmation of Registration and Payment
We will confirm receipt of your registration form within one week of the date we receive it. The confirmation, which will be sent to you via email, will include information about whether or not we have received your payment yet. Please note that we will not be able to save a spot for you at the conference until payment has been confirmed.
